EPSDT (Early and Periodic Screening, Diagnostic, and Treatment)

EPSDT is the federal Medicaid benefit that requires comprehensive and preventive health services for children under 21, including screening, dental, vision, hearing, and medically necessary treatment.

Definition.

EPSDT is the federal mandate that requires state Medicaid programs to cover any medically necessary service for Medicaid-eligible children under 21, even if the state does not cover that service for adults. ABA therapy for autism, behavioral health services, dental, vision, hearing, and specialty care all flow through EPSDT for pediatric Medicaid populations.

Key points.

Scope of EPSDT

Covers any medically necessary service for children under 21, including services that adult Medicaid does not cover. ABA therapy is covered under EPSDT in every state, though authorization rules and rate structures vary.

EPSDT and ABA

ABA therapy coverage for Medicaid-eligible children with autism spectrum disorder is federally required under EPSDT. State implementation varies widely in rate-setting, authorization, and managed care plan rules.

EPSDT periodicity schedule

Each state sets a periodicity schedule defining how often EPSDT screenings occur. Bright Futures schedules are the most common reference framework.

EPSDT denials and appeals

Denials of medically necessary services for under-21 Medicaid beneficiaries are challengeable under EPSDT. Successful appeals require clinical documentation of medical necessity.
