From ICD to AI. The evolution of medical coding.
Medical coding has gone through five eras since the 1970s. Each shift in technology forced a shift in workforce, workflow, and revenue cycle. Autonomous AI is the sixth era, and it is moving faster than the previous five combined.

Era 1, 1970s
ICD origins, paper coding
Hospitals used ICD 8 and ICD 9 on paper. Coders worked from medical records carts. Production was 30 to 40 charts per coder per day.
Era 2, 1990s
Electronic billing
Coding moved to encoder software. Submission moved to electronic claims. Productivity doubled. Manual coding stayed the dominant practice.
Era 3, 2000s
ICD 10 transition
The shift to ICD 10 expanded code count by 5x. Productivity dropped temporarily. Computer assisted coding tools emerged.
Era 4, 2010s
EHR integration
Coding moved into EHRs. Computer assisted coding hit majority adoption. Productivity rose, accuracy variable.
Era 5, early 2020s
Computer assisted plus rules
Hybrid CAC plus rule engine plus coder workflow. Accuracy stable. Productivity flat.
Era 6, 2025 forward
Autonomous AI
Domain LLMs read charts and code at human level on routine charts. Coders move to exception, governance, and audit roles.
