FQHC billing, PPS reconciliation, and revenue cycle services for California Federally Qualified Health Centers.
California operates 9,569 FQHC and Rural Health Clinic organizations registered in NPPES, representing 10.9 percent of the U.S. FQHC/RHC footprint. The four revenue streams of any FQHC, Medicaid PPS, FFS Commercial, FFS Medicare, and Self-Pay/Sliding, behave very differently in California: Medi-Cal pays PPS encounters at the rate letter; commercial payers pay against contracts; Medicare carve-outs (AWV, chronic care, BHI) pay FFS; sliding fee tracks against UDS Table 4 compliance. Generic RCM that treats FQHC like a private practice misses the PPS distortion. We do not.
FQHC revenue cycle management in California, in short
FQHC revenue cycle management in California runs on Medi-Cal rules: a clinic-specific PPS rate set by DHCS, managed care visits billed to the plan first and then to the Medi-Cal fiscal intermediary for the wrap, and an annual reconciliation with DHCS Audits and Investigations. ASP-RCM tracks each step per visit, along with scope-of-service requests, telehealth modifiers and associate-level behavioral health billing.

FQHC revenue cycle management in California: questions buyers ask
What does FQHC RCM in California cover?
It covers the Medi-Cal steps that sit on top of normal billing. Managed care members are billed to their plan, then a separate wrap claim goes to the Medi-Cal fiscal intermediary for the difference up to the clinic's PPS rate. Fee-for-service visits are billed per visit at that rate. Other coverage, including Medicare and commercial plans, is billed first because Medi-Cal is the payer of last resort. Payments are then reconciled each year with DHCS Audits and Investigations.
How is the Medi-Cal PPS rate for a California FQHC set and updated?
The DHCS Audits and Investigations Division sets each clinic's PPS rate, a fixed, all-inclusive per-visit rate. Under the California State Plan, the rate is adjusted every October 1 by the Medicare Economic Index increase for primary care services, and DHCS notifies each FQHC of the effect. Rates also move through scope-of-service change requests. Under PPS, California FQHCs are not required to file annual cost reports, so the rate letter and the reconciliation file carry the weight.
How do California FQHCs bill the Medi-Cal managed care wrap?
Bill the member's managed care plan first, even when the member is out of network or there is no contract. Then submit a separate claim to the Medi-Cal fiscal intermediary with revenue code 0521, HCPCS T1015 and modifier SE, listing the actual CPT or HCPCS codes on informational lines. Incident-to services are not eligible for the wrap. Plan payments are later trued up against the PPS rate through the annual reconciliation request to DHCS.
When must a California FQHC file a scope-of-service rate change?
Under the State Plan, a request can be filed once per fiscal year, within 150 days of the start of the fiscal year after the change occurred, and an approved change applies from the start of the fiscal year in which the request is submitted. The net rate change must reach 1.75 percent. A center must file if a scope decrease would lower its rate by more than 2.5 percent, or if it stops onsite pharmacy or dental services.
How do Medi-Cal telehealth rules apply to FQHC visits?
A visit can be face to face or by synchronous video, synchronous audio-only or asynchronous store and forward, billed with modifier 95, 93 or GQ. A new patient can be established by video or store and forward only when set conditions are met, including the patient being physically present at a licensed or intermittent clinic site. Audio-only cannot establish a patient unless the visit is a sensitive service, or the patient requests audio-only or has no video access. E-consults and remote patient monitoring are not reimbursable.

What good FQHC billing execution looks like for California providers.
The California FQHC billing market has its own quirks: Medi-Cal, dominant local Blue plan, regional commercial payer mix, and (where applicable) Tricare West. Here is the operating discipline we install on every California engagement.
- Four-stream rate report installed in monthly closeMedicaid PPS, FFS Commercial, FFS Medicare, and Self-Pay/Sliding tracked separately on a single page. California CFOs see realization per stream against the California-appropriate benchmark, not a blended GCR that hides the truth.
- PPS rate letter and wrap-around reconciliation disciplineCurrent Medi-Cal PPS rate letter, prior three years, and quarterly wrap settlement statements all in one folder, read by the RCM director on day one of every fiscal year. Wrap-around payments reconciled back to the originating MCO encounter, not posted as standalone receipts.
- Change-in-scope (CIS) calendarEvery service line change, new site, provider mix shift, or significant staffing model change triggers a CIS review for Medi-Cal PPS rate recalculation. California CIS approvals routinely add six-figure annualized PPS revenue.
- Commercial benchmark against Medicare regional fee scheduleCommercial payer allowables for California FQHC visits benchmarked monthly against Medicare regional fee schedule. Where commercial realization drops below 75 percent of Medicare allowed, it is a contracting and credentialing failure, not a billing failure.
- Three-checkpoint eligibility verificationMedi-Cal managed care enrollment shifts at every redetermination. Real-time eligibility at scheduling, at 72-hour confirmation, and at check-in catches plan changes that turn billable visits into write-offs.
- Sliding fee schedule accuracy at registrationFederal Poverty Level verification and sliding fee discount application at intake. UDS Table 4 reconciliation monthly. California HRSA audit-ready documentation.
- Reason-code denial work, not payer denial workTimely-filing denials across three Medi-Cal MCOs is a process problem, not three payer problems. Reason-code-first denial analytics surface systemic issues that payer-first work hides.
- AWV, BHI, and chronic care code capture for FFS Medicare streamAnnual Wellness Visit penetration tracked monthly. Behavioral Health Integration (BHI) and chronic care management (CCM) code utilization measured against eligible Medicare beneficiary panel.
