The Question · ABA · Transition to adult services
At 21, the Medicaid ABA benefit stops being guaranteed.
EPSDT makes states cover medically necessary ABA for Medicaid members under 21. After that, coverage depends on the state plan, a waiver slot or a commercial mandate with its own age cap. Indiana ended adult ABA on October 1, 2026. Here is who pays, and what to build now.

The question we asked on LinkedIn
Who funds ABA when a child turns 21 and EPSDT ends, and what should a provider build now?

Where funding can come from after the 21st birthday
Each step only exists if the state or plan offers it. Check them in this order.
EPSDT ends
The federal duty to cover any medically necessary 1905(a) service stops on the 21st birthday.
Adult state plan
Some states cover ABA for adults in their Medicaid state plan. Others, like Indiana now, do not.
HCBS waiver
CMS says adults can keep autism services through a 1915(c) waiver if the state plan does not cover them. Slots and waitlists vary.
Commercial plan
State autism mandates apply to fully insured plans. Many stop at 21 or younger.
No payer
Self-pay, or a planned discharge with community supports in place.

Funding routes for adult ABA
Federal rules set the frame. Each state decides what is actually covered.
| Route | Who it covers | What to check |
|---|---|---|
| EPSDT | Medicaid members under 21 | Medical necessity; states must cover it even if not in the adult plan |
| Medicaid state plan, adults | Adults, only where the state elected the service | State plan amendment, adult age limits, hour caps |
| 1915(c) or 1915(i) HCBS | People who meet the waiver's target group and level of care | Waiver service definitions, codes, rates, waitlist, case manager authorization |
| Fully insured commercial | Members of plans regulated by the state | State mandate age cap and annual limits |
| Self-funded employer plan | Employees of large self-insured employers | Plan document only; ERISA plans sit outside state mandates |
CMS described these Medicaid routes in its July 7, 2014 informational bulletin on autism services.
Commercial autism mandates with an age cap, selected states
From the NCSL summary of state autism insurance laws. Several other states have no age limit.
| State | Age limit in the mandate |
|---|---|
| Illinois, Louisiana, Pennsylvania | Under 21 |
| Georgia | 20 or under |
| Montana, West Virginia | Through 18 |
| South Carolina | Under 16, diagnosed at 8 or younger |
| Rhode Island | Until age 15 |
Mandates are amended often. Confirm the current statute and the plan type for each client before planning a transition.
Billing a child versus billing an adult
The CPT codes do not change at 21. The payer, the rules and the paperwork usually do.
Carrying the child setup forward
- Same payer on file after the 21st birthday
- Medical necessity written around school-age goals
- Authorization renewed on the old cycle
- Caregiver training billed with no caregiver named
Setting up the adult case
- Coverage re-verified before the birthday month
- Goals tied to adult daily living, work and safety
- Waiver service plan and codes loaded where the waiver pays
- Caregiver or residential staff named for 97156

EPSDT ends at 21 and nothing replaces it automatically. Run an 18-month age-out report, check the state plan, the waiver and the mandate age cap for each client, and switch payers before the birthday, not after the first denial.
Questions operators ask next
Does Medicaid cover ABA therapy after age 21?
It depends on the state. EPSDT requires states to cover medically necessary ABA for Medicaid members under 21. For adults, coverage exists only if the state added ABA to its adult state plan or offers it through a home and community based services waiver. Indiana, for example, stopped paying ABA for members 21 and older on October 1, 2026.
Do commercial insurance autism mandates have age limits?
Many do. According to the NCSL summary, Illinois, Louisiana and Pennsylvania mandates cover people under 21, Montana and West Virginia through 18, and South Carolina under 16. Other states have no age limit. Self-funded employer plans follow their plan document, not state mandates.
Can a 1915(c) waiver pay for ABA for adults?
Yes, where the state's waiver includes behavior services. CMS said in a July 2014 bulletin that individuals 21 and older may continue to receive autism services through a waiver if the state does not cover them for adults in its state plan. Eligibility, slots, codes and rates are set by each waiver.
What is the current autism prevalence according to the CDC?
The CDC's Autism and Developmental Disabilities Monitoring Network estimates that about 1 in 31 children aged 8 had been identified with autism in 2022. The report was published in April 2025. Those children turn 21 in the early 2030s, which is why adult funding is a planning issue now.
