CMS finalized a 2.3% ASC update for 2027 on November 4, 2026. Its own impact table shows eye ASC payments down 1% and cataract 66984 down 2%, while musculoskeletal and spine rise 6%. Here is why, which eye codes move, and what changes on drug and device lines.

If the ASC update is 2.3%, why does your cataract facility fee go down in 2027?

Why a 2.3% update turns into a cut for eye ASCs

CMS explains the mechanism in the final rule. Specialties built on device-intensive procedures gain. Specialties with few of them, like eye, lose.

  1. OPPS weights rise

The final 340B drug payment policy is offset by a budget-neutral increase in OPPS surgical payment rates.

  2. Device portions go up

ASC device portions of device-intensive procedures use the OPPS conversion factor, so they rise with OPPS.

  3. Scalar drops to 0.814

To keep ASC spending budget neutral, CMS scales down the non-device portions of all other ASC procedures.

  4. Eye codes absorb it

Procedures that are not device-intensive get a small increase or a decrease. CMS shows eye at -1% and 66984 at -2%.

Covered procedures list and quality reporting

618 codes added to the ASC list

Most come off the Inpatient Only list. The eye additions are new codes: 0730T laser trabeculotomy and 1084T suprachoroidal scaffold allograft.

ASCQR: no new eye measures

CMS finalized no ASCQR changes beyond removing the colonoscopy follow-up measure. Unplanned Anterior Vitrectomy and Wrong Site stay mandatory.

Cataract visual function stays voluntary

Cataracts: Improvement in Visual Function within 90 Days remains voluntary through the CY2032 payment determination. No penalty for not reporting it.

Miss ASCQR, lose 2 points

An ASC that fails reporting gets a 0.3% update instead of 2.3%, so its conversion factor is $56.592, about 2% lower on every Medicare case.

Questions operators ask next

What is the 2027 ASC conversion factor?

CMS finalized $57.721 for ASCs that meet quality reporting requirements. It is the 2026 factor of $56.322 times the 2.3% update and a wage index budget neutrality factor of 1.0018. ASCs that fail ASCQR get a 0.3% update, a conversion factor of $56.592.

Why do cataract ASC payments go down in 2027?

The ASC weight scalar fell from 0.872 to 0.814. CMS scales only the non-device portions of ASC rates, and device portions rose with the OPPS. Procedures that are not device-intensive absorb the difference. CMS estimates 66984 ASC payments fall 2% and the eye group falls 1%.

Which eye code gains in the 2027 ASC rule?

CMS estimates 66991, cataract surgery with insertion of an intraocular anterior segment drainage device, rises 9% in aggregate ASC payments. Most other high-volume eye codes fall, including 66984 and 66982 at 2%, 66821 at 3%, 65820 at 1% and 15823 at 5%. These are aggregate CMS estimates, so check each code in Addendum AA.

When does iDose TR pass-through end?

The final rule lists pass-through for J7355 through June 30, 2027. After that the drug is no longer paid separately under pass-through. A commenter said its cost exceeds the APC 5492 payment for 0660T. CMS kept 0660T at status J1 and will decide J7355's later status separately.
