On 1 January 2027, some Medi-Cal eye patients change payers overnight.

Members with unsatisfactory immigration status leave Medi-Cal plans for fee-for-service. Expansion adults face work rules and renewals every six months. For an eye practice, that means new payers, new authorization rules and more eligibility gaps. Here is what changes, when, and what billing has to do.

Routing a UIS patient's eye visit from 1 January 2027

  1. Check eligibility for the date of service

Confirm full-scope Medi-Cal and the delivery system. A plan card will not work for these members.

  2. Confirm FFS enrollment

Every rendering optometrist and ophthalmologist needs an active Medi-Cal FFS enrollment, not only a plan contract.

  3. Get a TAR where FFS needs one

For example, contact lens fittings 92071, 92072 and 92310 to 92312 need a TAR from the DHCS Vision Services Branch.

  4. Bill Medi-Cal FFS

Original claims within six months after the month of service for full payment.

  5. Follow up with CIFs and appeals

Use the FFS clocks below, not the plan's dispute process.

Prop 35 supplemental payments: read the fine print

A fixed window

Dates of service 1 July to 31 December 2026, pending federal approval of SPA 26-0029.

Check the provider type

DHCS lists physicians, PAs, NPs, podiatrists and other types. Optometrists are not on the published list of eligible provider types.

FFS and plan networks

Eligible providers in FFS and network providers of Medi-Cal plans qualify, for listed codes billed on the CMS-1500.

Enrollment timing for new eye providers

DHCS has up to 180 days to act on a PAVE application. A plan can let a pending provider see members for only 120 days.

Start PAVE on the start date

  • Plan network participation can stop at day 120 if DHCS has not acted
  • No FFS enrollment for UIS patients after 1 Jan 2027
  • Claims pile up on hold while enrollment is pending

Start PAVE at the offer letter

  • FFS enrollment approved before the first clinic day
  • Plan contracting runs in parallel
  • Every location the provider works at is on the application

The Medi-Cal provider manual covers a routine comprehensive eye exam, 92004 for new and 92014 for established patients, once every two years for recipients of any age, billed by an ophthalmologist or optometrist. DHCS says more frequent exams are covered when medically necessary, such as for eye pain or blurred vision. Eyeglasses follow a 24-month cycle.

What is the deadline for a Medi-Cal fee-for-service appeal?

An appeal must be submitted within 90 days of the action or inaction that caused the complaint, under 22 CCR 51015. Late appeals are denied.
