How the process runs

Hospital OPD prior authorization, step by step

Identify the service at scheduling

Step 1

The scheduling edit fires on the listed code and the hospital outpatient setting, before the appointment is confirmed.

Assemble the documentation

Step 2

Everything needed to show the service meets Medicare coverage, coding and payment rules, including the local coverage determination criteria.

Submit before the service

Step 3

The request goes to the MAC before the service is furnished and before the claim is submitted.

MAC decision

Step 4

  • Standard: 7 calendar days for requests submitted on or after January 1, 2025
  • Expedited: 2 business days [3]

Provisional affirmation or non-affirmation

Step 5

An affirmed request can be billed; a non-affirmed request can be resubmitted with additional documentation.

Claim submission

Step 6

The claim carries the decision reference; a claim for a listed service without a decision is not paid.

Takeaway

The scheduling edit is the whole program. If the code is caught at scheduling, the 7-day clock runs before the visit; if it is caught at coding, the visit has already happened without authorization.

FAQ

Frequently asked questions

When does prior authorization start for the additional botulinum toxin codes?

If finalized as proposed, for dates of service on or after July 1, 2027, in the hospital outpatient department setting. The CY2027 OPPS rule is still proposed; CMS expects to finalize it in November 2026.

Which botulinum toxin codes already require prior authorization in the hospital outpatient department?

CPT 64612 and 64615, and HCPCS J0585, J0586, J0587 and J0588, since July 1, 2020. The proposed rule adds eight more codes listed in Table 76 of the rule.

How long does the MAC take to decide an OPD prior authorization request?

Seven calendar days for standard requests submitted on or after January 1, 2025, and two business days for expedited requests.

Why is CMS expanding the category?

CMS reports claim volume for the Table 76 codes grew 42.8 percent from 2017 to 2024 while total OPD claims fell 8.3 percent, and it estimates about $25 million in annual Medicare savings from requiring prior authorization.
