ABA ยท Growth decisions

The decisions you are not making today. Is your back office the reason?

Most ABA owners can name the state they would open, the payer they would add and the families on a waitlist. What holds them back is often a question no one can answer quickly: how long until a new clinician can bill, what does this payer really pay, and who will chase it? Here is what has to be true for each decision.

The question we are asking on LinkedIn

What is the last growth decision you put off because your back office could not give you a straight answer on timing, rates or rules?

Six decisions, and what each one needs

Open a new state

State Medicaid enrollment, each managed care plan and each commercial payer, run in parallel where the state allows. See how serial Texas is.

Add a payer

The enrollment path, the contracted rate by code, and the authorization rules, known before you promise an opening to a family.

Accept TRICARE

TRICARE runs ABA through its own demonstration, with its own referral, authorization and documentation rules. It is a program, not one more payer.

Hire five clinicians

Where each new hire is with each payer, on one screen, so a start date is not a guess and no one has to track them down.

Add RBTs to the roster

The rate for 97153 by payer, against what an RBT costs you with supervision. See what RBT pay and reimbursement look like.

Add a 2027 service line

Harmful behavior services and 97180 depend on staffing, documentation and whether the payer covers them. See the 2027 codes.

TRICARE, in the rules that matter to your calendar

Autism Care Demonstration (ACD), per TRICARE as of its September 2026 page.

RuleWhat it means operationally
Authorized through Dec 31, 2028The ACD is a demonstration with an end date. Plan on it, and watch for changes.
Referral and pre-authorization requiredAll TRICARE plans need both before ABA services begin.
Six-month authorizationsThe care plan and outcome measures have to be current at each renewal.
New referral every 24 monthsTrack it by client, or you find out when the authorization stalls.
West Region runs through TriWestCertification, a participation agreement, a network agreement and credentialing each change what you can do. Check your regional contractor.

Before you say yes to a new state

  1. Map the payers

List Medicaid, each managed care plan and each commercial payer, with its enrollment path and how many of your families it covers.

  2. Check who can bill

Licensure, what each payer accepts for BCBA, BCaBA and RBT, and the modifiers each one expects.

  3. Price the rates

Contracted rate by code against your cost to deliver, including 97155 and 97173 in 2027.

  4. File in parallel

File early and in parallel where you can. Some states run in sequence, and that alone can take months.

  5. Set the start date last

Schedule families against the first payer that is actually live, not the first application filed.

The same decision, with and without visibility

Held back by the back office

  • "Credentialing says soon"
  • Rates known after the first remit
  • New hires sitting unbilled, and no one sure why
  • Growth waits for a free week

Made with visibility

  • Every application with an owner and a follow-up date
  • Rates and rules known before the contract is signed
  • Each new clinician's status on one screen
  • Growth is a decision you schedule

TAKEAWAY

The back office should never be the reason you say no to growth. If it is, the fix is visibility and ownership.

Questions operators ask next

How long does it take to credential a BCBA in a new state?

It depends on the state and payer. Industry cycles often run 90 to 120 days per payer, and some Medicaid programs run in sequence and take months: Texas can take five to seven months to reach full managed care panels. Plan your start dates around the first payer that is live.

How does TRICARE cover ABA?

Through the Autism Care Demonstration, which TRICARE says is authorized through December 31, 2028. All plans need a referral and pre-authorization, authorizations run six months, and a new referral is needed every 24 months. Requirements for providers run through the regional contractor.

How do we decide whether to add RBTs?

Compare the 97153 rate for each payer with what an RBT hour costs you, including the BACB's minimum of 5% supervision each month, plus cancellations. Also check what each payer requires of technicians, because some enroll or register them and others bill under the supervising clinician.
