IDR batching changes on November 1: the 50-line cap, and what moves with it

Short answer: a batched Federal IDR dispute whose open negotiation starts on or after November 1, 2026 is limited to 50 line items with a 30-business-day cooling-off. Gateway filing opens November 13, web forms close after January 15, 2027, and from January 1, 2027 the remit says whether a claim can go to IDR.

Frequently asked questions

When does the 50-line batching cap take effect?

For batched disputes whose open negotiation period begins on or after November 1, 2026, per CMS's August 7, 2026 timeline guide; the Notice of IDR Initiation form enforces it from that date.

What is the new cooling-off period for batched disputes?

30 business days after a payment determination for batched disputes started on or after November 1, 2026. Earlier batches and all single and bundled disputes keep 90 calendar days.

Do I need an IDR Gateway account if my billing company files for me?

No. CMS states that parties using a third-party administrator only need that administrator to sign up. Confirm in writing before November 13, 2026.
