ABA billing for multiple locations: one model for every site

Short answer: multi-location ABA groups lose money the same way: each site keeps its own payer rules, unit counting and denial pile. The fix is five shared objects, the 2026 state rules they carry, and a ledger built for the prior authorization API on January 1, 2027.

Why ABA billing breaks at the third location

Three sites, three versions of every rule, and no single report that explains the drift

One clinic runs on a spreadsheet and a good office manager. By the third location that produces three versions of every payer rule, three ways of counting units and three denial piles nobody reconciles. ABA is authorized in 15-minute units under modifier, credential and telehealth rules that differ by state and payer. When each site owns its own rules, the group has no rules.

1 payer matrix

Every state and payer rule the group bills under, dated and sourced, loaded once.

1 auth ledger

Every authorization in 15-minute units, matched to sessions and claim lines.

1 enrollment roster

Every BCBA, BCaBA and RBT, every NPI, every payer effective date.

1 denial queue

Every denial by root cause and owner, not by site.

1 remit reconciliation

Every 835 line to its ledger row and dated rate; cash per delivered hour by site.

Takeaway

Centralizing ABA billing does not mean one office doing everything. It means one set of rules, one ledger and one queue, with the work assigned wherever the people are.

Object 1: the payer matrix

State rules diverged in 2026; a group in four states holds all of them at once

Several 2026 rules turn a documentation or modifier gap into a denied unit. The matrix is maintained centrally and read by every scrubber rule and reauthorization packet; each row carries the rule, source, effective date and control. A rule with no control is a memo. Our public ABA payer policy matrix is the starting dataset.

Object 2: the authorization ledger

The heart of the model, and the API that arrives on January 1, 2027

Every authorization is a row: member, payer, codes, units authorized, dates, plan type, unit convention. Sessions post as delivered units, claim lines as billed units; the gap is the leakage report. Under CMS-0057-F, impacted payers must decide in 72 hours expedited and seven calendar days standard with a specific denial reason, generally since January 1, 2026, and from January 1, 2027 run a Prior Authorization API that identifies documentation requirements and returns an approval with its end date, a denial with a reason, or a request for more information [2].

Takeaway

Centralize the ledger first. It is the one object that every site, every payer and every denial already touches.

Object 3: the enrollment roster

One row per credential per payer: in 2026 the roster became a condition of payment

Georgia

Jul 1, 2026 revalidation

  • Missed revalidation: later dates of service unpaid
  • New effective date is the submission date, no retro-enrollment
  • Termination 30 days after the suspension letter if nothing is filed [6]

North Carolina

  • Paraprofessionals must hold RBT or ABAT certification to be reimbursed

Indiana

Accreditation

  • Center-based MA and RI providers by January 1, 2027; all others by January 1, 2028

1

One roster across every entity and site

Billing NPI, rendering NPI, taxonomy, certification number and expiry, and each payer's effective and revalidation dates.

2

Run it against the state revalidation calendarmonthly

And against the certification registry before the first claim for any new RBT or BCaBA.

3

Block scheduling where enrollment is pending

Or route those sessions to a held-claims report with a release date.

4

Credentialing as a timed pipelineowner per application

Our BCBA credentialing service runs on CredPro, our credentialing platform: document intake, parallel payer submission and effective-date capture on one record. A slow enrollment costs what our 22-day BCBA credentialing case study shows; a lapsed one, after Georgia's July 2026 deadline, costs every unit billed until fixed.

Object 4: the denial queue

One queue

Site-level denial work produces heroics and no learning. A central queue by root cause, payer and owner turns the same denials into template fixes. Each item carries the CARC and RARC codes, the ledger row, the rendering credential and the matrix rule missed, so the owner fixes the control, not the claim.

Reauthorization not supportedPlan above 16 hours a week without three-month reauth in North Carolina; Indiana FBA missing at reauthClinical documentation lead
How we run itWork Genie, our AR and denial workflow tool, is built around exactly this: one queue, one owner per root cause, every touch recorded, and a payer prior-authorization form library behind the authorization work. The three-way match recovery case study shows a 42-site chain reversing $1.4M of takebacks once every claim was gated on authorization, supervision ratio and the session note.
Takeaway

Measure the queue by root causes closed, not by claims touched. A root cause closed stops denying at every site at once.

Object 5: remit reconciliation

Cash per delivered hour by site: the number that tells leadership whether the model works

Every remittance line reconciles to the ledger row, the dated contract rate and the date of service; variances are underpayments to escalate or defects to fix.

Cash per delivered hour

By site, payer, code

The number that compares locations fairly after rate cuts.

Authorization utilization

Weekly

Delivered units over authorized units by site and BCBA.

Remit-to-schedule variance

By payer

Paid amount versus the dated fee schedule or contract.

Sessions without a line

Five business days

Delivered sessions with no billed claim line.

1

Post every 835 line to its ledger row

By member, date of service, code and modifier; lines that find no row are the first exception report.

2

Compare paid to the dated schedule or contract30 days to escalate

Variances above a set threshold go to a written payer escalation.

3

Roll up by site, payer and codemonthly

Delivered hours, paid dollars, denied units; publish cash per delivered hour as the one comparison metric.

4

Feed every variance root cause back

Into the matrix, the roster or the scrubber so the same line does not reappear next month. Eligibility sits upstream: a 270/271 check before the first session of each authorization period, including the benefit and plan-level caps; CorePulse, our eligibility and benefit verification engine, runs it as one transaction per member. Read site metrics on contract value, not gross charges: gross collection rate guide.

FAQ

Frequently asked questions

What is the biggest billing risk when an ABA group adds locations?

Rule drift. Each site develops its own version of the payer rules, unit counting and denial handling, so modifier, credential and authorization defects multiply with every location. A single dated payer matrix, one authorization ledger and one denial queue remove that drift without moving staff.

Do multi-location ABA groups need to centralize billing staff to centralize billing?

No. The model centralizes the objects, meaning the payer matrix, the authorization ledger, the enrollment roster, the denial queue and remit reconciliation. Work can be assigned to people at any site as long as they operate from the shared objects and the root-cause owners are named.

How does CMS-0057-F affect ABA prior authorization in 2027?

From January 1, 2027, Medicare Advantage organizations, Medicaid and CHIP fee-for-service programs, Medicaid and CHIP managed care plans and QHP issuers on the federal exchanges must run a Prior Authorization API that identifies documentation requirements and returns an approval with its end date, a denial with a specific reason, or a request for more information. Decision timeframes of 72 hours expedited and seven calendar days standard generally began January 1, 2026. Drug prior authorizations are excluded.

Which 2026 state rules most often cause ABA denials across sites?

Indiana's credential, group-size and UA modifiers and its telehealth restrictions; North Carolina's RBT or ABAT certification requirement, telehealth removal for 97152 to 97154 and three-month reauthorization above 16 hours a week; Georgia's July 1, 2026 revalidation deadline; and Missouri's one-line-per-code-per-day unit rule. Each should be a scrubber or scheduling control, not a memo.

What single metric best compares ABA locations after a rate cut?

Cash per delivered hour by site, payer and code, read alongside authorization utilization. It separates a site with a weak payer mix from a site that is leaking authorized units or losing delivered units to denials.
