Lifetime allocation
Modifier UA
- Up to 4,000 hours (16,000 units) of comprehensive ABA per member; UA on the PA request and the claim
- 97155 and 97156 excluded, never UA
- Targeted ABA: up to 15 hours a week, no UA, after the 4,000 hours
Indiana bundled a phased cut with new modifiers, caps and supervision rules; New York cut one code for one credential; Nebraska rebuilt its schedule on neighbors' average. Every action lands on the 15-minute unit, so the defense is built unit by unit.
| State | Instrument | What changed | Dates of service |
|---|---|---|---|
| Indiana | IHCP bulletin BT202627 [1] | 6% maximum fee reduction on all nongroup ABA codes, then an additional 4% on every ABA code including group services | Apr 1, 2026 Apr 1, 2027 |
| New York | Medicaid Update, August 2026 [2] and SPA 25-0061 [3] | Fee-for-service rate for CPT 97153 delivered by unlicensed individuals or technicians reduced to $9.63 per unit under the FY 2026-27 Enacted Budget | On or after Oct 1, 2026 |
| Nebraska | SPA NE-25-0020 [4] | ABA rates adjusted to a blended average of nearby states' Medicaid ABA rates; same rates for governmental and private providers; per-code amounts in the DHHS fee schedule the SPA points to | Effective Aug 1, 2025 CMS approved Aug 6, 2026 |
| Massachusetts | 101 CMR 358.00 hearing notice [8] | EOHHS proposes to maintain current ABA rates, treated as no change in aggregate spending for 197 providers | On or after Dec 1, 2026, subject to federal approval |
A rate cut is a change to the price of a unit. Everything you control sits on the other side of that multiplication: how many authorized units you deliver, how many you bill correctly the first time, and which payers those units belong to.
BT202627 bundles a rate cut with program-integrity rules; the numbers are the bulletin's own tables [1].
| Code and modifier | Through Mar 31, 2026 | DOS from Apr 1, 2026 | DOS from Apr 1, 2027 |
|---|---|---|---|
| 97153 U1 (RBT) | $17.06 | $16.04 | $15.39 |
| 97153 U2 (BCaBA) and U3 (BCBA, BCBA-D, HSPP) | Not payable | $16.04 | $15.39 |
| 97155 U3 (BCBA protocol modification) | $27.63 | $25.97 | $24.93 |
| 97155 U2 (BCaBA) | $21.85 | $20.54 | $19.72 |
| 97151 U3 (assessment) | $27.63 | $25.97 | $24.93 |
| 97156 U3 (caregiver guidance) | $28.23 | $26.54 | $25.47 |
| 0362T and 0373T U1 | $28.54 | $26.83 | $25.75 |

Unit-killing rules
In Indiana the rate cut is 6% then 4%. The unit loss from a missed UA modifier, a wrong group tier, a telehealth 97153 line or an unsupported BIP is 100% of the unit.
New York's cut is narrow and deep: 97153 by unlicensed individuals or technicians to $9.63 per unit from October 1, 2026 [2]; SPA 25-0061, transmitted December 31, 2025, describes the same reduction [3]. Nebraska's SPA NE-25-0020 aligns rates, effective August 1, 2025, with a blended average of geographically proximate states, approved August 6, 2026 [4]; reconcile every 2026 remittance against the DHHS schedule, not 2024 contract loads.
The same RBT hour is worth $38.52 to $64.16 depending on the state [1] [7] [2].
A 10% cut is offset if clean units rise about 11% on the same payer, or hours shift to a payer still paying the old rate. Neither happens by accident.
Set by bulletin or contract; the column you cannot move this quarter.
Authorized units delivered, documented and paid first time.
Who owns the delivered hours, at what rate after denials.
Illustrative arithmetic on bulletin rates; percentages are your own inputs [1].
A six-point gain in authorized-unit delivery more than covers the 4% second-phase cut; a 3% defect rate gives most of it back. Model your own payers before April 1, 2027.
Cancellations, open RBT hours, late treatment plans and unassigned caseloads show up as units that expire. At scale: our multi-state diagnostic, where $5.05M of claims were never answered.
Every auth by member, payer, code, units authorized, scheduled, delivered and billed; end date; unit convention per payer.
Delivered over authorized units; flag any member below the clinic's floor.
The second group needs a discharge or reauthorization decision, not scheduling.
A session with no line is a defect, not a backlog.
Indiana PA requests must include caregiver coaching hours, so unbilled 97156 is lost revenue and a compliance gap [1].
Unit conversion is its own leak. MO HealthNet's September 9, 2026 hot tip: exact start and stop times, subtract naps, meals and breaks, sum active minutes per code per day, convert once, one line per code per day; duplicates deny in eMOMED [5].
| Total active minutes in the day, per code | Units to bill |
|---|---|
| Fewer than 8 | 0 |
| 8 to 22 | 1 |
| 23 to 37 | 2 |
| 38 to 52 | 3 |
| 53 to 67 | 4 |
| 68 to 82 | 5 |
| Each additional 15-minute increment | +1 when at least 8 minutes are reached |
After a cut, every denied or recouped unit costs the same rework and returns less cash. These defects were published in 2026 bulletins, so payers are checking for them.
| State | Rule | What the claim must carry |
|---|---|---|
| Indiana [1] | Credential modifiers U1, U2, U3; group-size modifiers U4, U6, U8; UA for comprehensive services; rendering specialty 625, 624 or 615 | The modifier set that matches the rendering credential, the group size and the plan type on the PA |
| Indiana [6] | FBA required for each BIP target behavior from October 29, 2026, refreshed at each reauthorization | A treatment plan that will survive a reauthorization review |
| North Carolina [12] | Paraprofessionals need RBT or ABAT certification, 120-day grace from August 1, 2026; no telehealth for 97152 to 97154; plans above 16 hours a week reauthorized every three months; at least 10% of paraprofessional hours observed by the LQASP | A credentialed rendering technician and a supervision log that supports the 10% |
| Georgia [11] | Providers who missed the July 1, 2026 revalidation have claims with later dates of service unpaid; the new effective date is the submission date, with no retro-enrollment | An enrollment that is current at every billing and rendering NPI |
| Missouri [5] | One line per code per day, exact start and stop times, no block billing | A daily unit total per code that reconciles to the session record |
Credential, group-size and plan-type modifiers; telehealth rules by code.
Before the first claim and at every revalidation date.
97152 to 97154 with place of service 02 or 10 where the state removed telehealth.
Indiana's one-in-eight rule and North Carolina's 10% observation rule are the audit baseline.
Fix the template, not the claim; a 3% defect rate at a lower price is the fastest way to lose recovered margin.
Clean-claim work after a rate cut is not a cost center. It is the only lever that returns cash without adding a single clinical hour.
Some 2026 commercial rules give ABA providers more room than they realize. Where an MCO pays below the state schedule, ask for the contract clause and the state rate floor in writing before the next amendment; the ABA payer policy matrix tracks these notices.

The groups that hold margin through 2027 will not be the ones that negotiated hardest. They will be the ones that delivered and billed the units they were already authorized to provide.
IHCP bulletin BT202627 applies a 6% maximum fee reduction to all nongroup ABA codes for dates of service on or after April 1, 2026, and an additional 4% reduction to all ABA codes, including group services, for dates of service on or after April 1, 2027. For example, 97153 billed by an RBT moves from $17.06 to $16.04 and then to $15.39 per 15-minute unit.
The August 2026 New York State Medicaid Update states that, as authorized by the FY 2026-27 Enacted Budget, the fee-for-service reimbursement for CPT 97153 delivered by unlicensed individuals or technicians is reduced to $9.63 per unit effective October 1, 2026. Managed care plans set their own contracted rates.
State Plan Amendment NE-25-0020, approved by CMS on August 6, 2026 with an effective date of August 1, 2025, adjusts Nebraska Medicaid's ABA rates to a blended average of Medicaid ABA rates in nearby states, with the same rates for governmental and private providers. The per-code amounts are on the DHHS fee schedule page the SPA cites.
Deliver and bill more of the units you are already authorized to provide, then stop losing delivered units to modifier, credential, telehealth and unit-conversion defects. Payer-mix and contract work matters but takes months; unit capture and clean claims can move within a quarter.
No. The allocation covers comprehensive ABA services billed with modifier UA. CPT 97155 and 97156 are excluded and must not carry UA on the PA request or the claim, and targeted ABA of up to 15 hours per week is outside the allocation. School corporations are also excluded.
